By disclosing Information to us, you acknowledge and authorise us and/or other companies authorised by us to discuss relevant aspects of your medical condition(s), treatment or examination with the provider(s) of your treatment or examination and you authorise us to send your information, including your health and financial information, to third parties in connection with the administration of your membership.

We record telephone calls to monitor the quality of our response and for training purposes.

The information we collect is not sold or rented by us to others but we may share such information with authorised third parties in connection with the administration of your membership or for our personal market research purposes or with anyone we have a non disclosure agreement.

As a data subject you have the right to access and inspect a copy of most of the Information we hold that you have supplied and to ask for inaccurate Information to be corrected. In order to review the Information we hold about you, write to us at the address below enclosing a cheque for £10.00, payable to Dentists’ Provident Society Limited, to cover our administrative costs in dealing with your request:

We will always verify your identity before providing you with details of any Information we may hold about you.

You can ask us to update or remove your Information by writing to our member services team at the address above. If you submit a request to amend your data, we will investigate and if we agree we will correct our records.
